Bouncy House Participation Waiver

This waiver is required only for participants using the bouncy house.
In consideration for being permitted to use the bouncy house at the church event, I, for myself and/or as parent/guardian of the participant, acknowledge and agree to the following:
Assumption of Risk
I understand that bouncy house activities involve risks including jumping, bouncing, collisions with other participants, and falls. I voluntarily assume these risks for myself or my child.
Release of Liability
I release and hold harmless Trinity United Methodist Church, its directors, officers, employees, volunteers, and agents from any claims, damages, or injuries that may occur while using the bouncy house.
Third-Party Equipment
I understand that the bouncy house is provided by an independent third-party vendor and that Trinity United Methodist Church is not responsible for the equipment's condition, maintenance, or operation. Any claims related to the equipment should be directed to the vendor.
Medical Treatment
I consent to emergency medical treatment if needed and understand that I am responsible for any medical expenses incurred.
Physical Condition
I certify that the participant is in good physical condition and able to safely participate in bouncy house activities.
Rules and Supervision
I agree that the participant will follow all posted rules and instructions from church volunteers. I understand that while supervision is provided, the church cannot provide continuous one-on-one monitoring of each participant.
I certify that I have read and understand this document. I am 18 years of age or older. If signing for a minor, I am the parent or legal guardian with authority to execute this waiver.
